Who to contact
Oakwell Medical, Main Street, Blessington, Co. Wicklow is responsible for the use of information submitted through this website. For privacy questions or to exercise your rights, email reception@oakwell.ie.
Information you provide
A new patient registration request asks for your full name, date of birth, phone number, email address and home address. A general enquiry asks for your name, phone number, email address and a comment of up to 100 characters. We receive the information directly from you.
Please do not include symptoms, medical history, PPS numbers or other clinical information in the comment box. This website is not an emergency or clinical advice service. Sending a registration request does not confirm acceptance as a patient.
Why we use your information
We use registration details to review your request, identify and contact you, and arrange the next steps towards registration. We use enquiry details to respond to your message. Required fields allow us to process the request; if you prefer not to use the form, contact reception by email to discuss another way to get in touch.
Legal basis
For steps you ask us to take towards providing a service, including considering a registration request, the proposed basis is Article 6(1)(b) GDPR. For general correspondence and website security, the proposed basis is our legitimate interest in responding to enquiries and operating a secure service under Article 6(1)(f). The practice must confirm these bases against its actual arrangements before this notice is finalised. Any health information requiring special protection needs an appropriate additional basis and is outside the intended scope of the general enquiry form.
Who receives information
Submissions are processed and stored through Netlify Forms and notifications are sent to reception@oakwell.ie through our email service. Relevant practice staff and service providers supporting these systems may process the information for these purposes. Hosting systems may also process technical information such as IP addresses and request logs for delivery, security and spam prevention.
Netlify and Google operate internationally. Provider locations, any transfers outside the European Economic Area and the applicable safeguards, together with the practice’s email-provider arrangements, need to be confirmed by the practice before public launch.
How long information is kept
Retention should be limited to the time needed to handle your enquiry or registration request, complete any necessary follow-up and meet applicable record-keeping obligations. The practice must confirm its retention and deletion arrangements for Netlify submissions and mailbox copies. Information transferred into a patient record is subject to the practice’s separate patient-record privacy and retention arrangements.
Your rights
Depending on the circumstances and legal basis, you may request access, correction, erasure, restriction or portability of your personal information, and object to processing based on legitimate interests. Where processing relies on consent, you may withdraw it without affecting earlier lawful processing. Contact reception using the email above. You may also complain to Ireland’s Data Protection Commission.
Google Maps
The map on our Contact page is not loaded until you choose “Load Google Map”. Loading it connects your browser to Google, which receives technical information including your IP address and may use cookies or similar technologies. See Google’s Privacy Policy. You can choose not to load the map and still use the website. “Close Google Map” removes the map; it cannot undo information already sent to Google or remove cookies Google has already set. The separate Google Maps link opens Google’s website.
Cookies and website tracking
The website code does not add advertising or analytics trackers, or save your form details in browser storage. Your choice to load the map is not remembered between visits. Hosting, security services and the optional Google map may use their own technologies. The final hosting configuration should be checked before launch and this notice updated to describe any additional cookies or services.
